Pelvic recurrence after definitive surgery for locally advanced rectal cancer: a retrospective investigation of implications for precision radiotherapy field design

چکیده
Background To analyze the local distribution of pelvic recurrence after total mesorectal excision, with a view to simplifying the formulation of optimal individualized radiotherapy plans. Methods We retrospectively investigated the data of 168 patients diagnosed with recurrent pelvic cancer treated at Fudan University Shanghai Cancer Center between January 2008 and December 2012. The following were collected depending on availability: operative report, histological report, specimen photographs, initial preoperative images, images confirming local recurrence, and clinical history. Results A total of 203 lesions of local recurrence were identified. The most common sites of pelvic recurrence were the mesorectum, including the anastomotic stoma in 53.0% of cases; presacral space in 27.4%, and pelvic floor and perineum in 21.4% the cases. Recurrence was most common in the lower pelvic region (i.e., below the upper border of the acetabulum), accounting for approximately 76.2% (128 cases) of cases. In patients with mid-rectal and distal rectal carcinoma. Conclusions Patients with pelvic cancer may benefit by individualized treatment plans aimed at achieving a balance between tumor control and minimal risk of irradiation-induced toxicity.

Background: Purpose: The purpose of our study is to demonstrate that intensified neoadjuvant chemo-radiotherapy (CRT) treatment in locally advanced rectal cancer (LARC), aimed at further enhancing the complete pathological response and local disease control, is feasible and well tolerated. Materials and Methods: From January 2011 to December 2015, 62 patients (women 21, men 41, mean age 61,5, ...

Background: To determine which radiotherapy parameters are associated with the tumor response of locally advanced non-small cell lung cancer (NSCLC) patients undergoing concurrent chemoradiotherapy. Materials and Methods: Thirty one patients with IIIA/IIIB NSCLC underwent chemoradiotherapy with a median dose of 63 Gy. On our actual treatments, we made radiotherapy planning to cover the planning...
متن کاملNew drug in neoadjuvant chemoradiation for rectal cancer
Background: In recent years, neoadjuvant chemoradiation and subsequent surgical resection with total mesorectal excision has been shown to increase local control with decreased toxicity. Neoadjuvant chemoradiotherapy is the standard treatment for locally advanced rectal cancer. In this study we evaluated the efficacy a cox-2 inhibitor on pathologic response, sphincter preservation and acute tox...
متن کاملOptimal timing of computed tomography for assessing lymph nodes after neoadjuvant chemoradiotherapy for rectal cancer
Background: This study assessed the optimal timing of computed tomography for detection of metastatic disease in locoregional lymph nodes in patients with rectal cancer who have undergone chemoradiotherapy. Materials and Methods: This observational retrospective study was performed in a single institution. All patients with locally advanced rectal cancer treated with chemoradiotherapy, followed...
متن کاملOutcomes of salvage surgery for cure in patients with locally recurrent disease after local excision of rectal cancer.
BACKGROUND Local excision for early rectal cancer has gained widespread interest. Currently available imaging modalities have low sensitivity to detect locoregional disease, which may result in understaging and a high risk of recurrence after local excision. OBJECTIVE The purpose of this work was to study the morbidity, mortality, and long-term oncologic outcomes in a select cohort of patient...
